We are a software house that works with the same clients over many years. Some of those relationships run past 15 years. The systems we build sit in production and have to stay available. The work splits between caring for existing systems and building new ones.
Teams are small, so each person owns a real part of a product or a client. New people start inside existing code, learn the domain and talk to the client directly. Work against sensitive production environments is orderly, with code review and a clear code standard.
